2025 Monthly Meeting Registration Form

    All members must register for each month that you are attending, even if you have paid the bundle rate.This is to ensure we have an accurate head count for space - including chairs and tables - and for the breakfast. 

    To register for a monthly program, please complete this registration form and indicate which month you plan to attend.  If you are completing a registration for someone else, please use the name of the person attending and NOT your name.  Use the email address where you would like the invoice sent.  Each attendee, including guests, must register separately.

    Please only submit this page one time.  For those who haven't bundled and are paying per meeting, you will be invoiced for attendance after the meeting via an invoice from QuickBooks sent to the email address you used when registering. You will  be able to pay this invoice via credit card online or by printing the invoice and sending a check to the address included on the invoice.

    NOTE: If you are a member of SHRM, this email address must match the email address on file with SHRM!
    Non-SHRM member fee per meeting is $40
    Documentation of your attendance will be emailed to you after the meeting, if requested.
    By choosing “Yes” you will automatically be included in a drawing for a prize if the Presenter or Sponsor provides one.
